I hereby apply for the DMO Service Agreement and corresponding insurance coverage via DMO, acting as the insurance intermediary, as follows: For Modules 1, 2, and 4, an insurance contract with Allianz Global Corporate & Specialty SE; and, with respect to Module 3, confirmation of the existence of legal protection insurance in my favor, in accordance with the terms of a legal protection insurance contract between the Förderverein für Modellsportfreunde e.V. and Allianz Versicherungs AG, Munich. DMO is authorized by the insurer to receive premiums on behalf of and for the account of the insurer. Receipt by DMO is considered receipt by the insurer. The applicant authorizes the German Model Sports Organization to debit the due service fees by direct debit until further notice from:

I expressly agree that insurance coverage shall commence prior to the expiration of my 14-day withdrawal period (see the instructions on the right of withdrawal at the end of the application form).
